I am looking for a certain mod.

Hello people, I am looking for a certain mod that I usually play Halo on PC. I want to download this mod so I can host my own game. Hopefully someone this forum knows what I am talking about. The mod allows the map to change after the round of CTF is done. It also uses heath packs to transport you to another parts of the map.
